For the Credit Union to make deductions from your paycheck, you need to complete a Deduction Authorization form.  Upon completion of the form, simply print it, sign it, and submit it to your credit union representative.

To apply for a loan with the credit union, you must complete a Loan Application form.  Upon completion of the form, simply print it, sign it, and submit it to your credit union representative.
